Age Requirement

The Site and our services are not directed to persons under 13 years of age. Users must be at least 13 years old to use the Site. For users under 18, a parent or legal guardian must provide consent for use and any information submission.

Cancellation, No-Show, and Late Arrival Policy

Please arrive at least 15 minutes before your appointment start time.

Out of respect and consideration of our other clients and our Specialists, we kindly ask for a minimum of 24 hours advanced notification to change or cancel a single service. A 48-hour minimum cancellation is required for multiple services scheduled for the same day.

A credit card is required at time of scheduling services. We reserve the right to charge up to 100% of the price of service scheduled for any late cancellations (under the 24-hour minimum, prior to appointment). Our policy is designed out of respect to our other guests, as well as compensation to our employees who depend on us to manage their schedules and support their careers with Wellness Origin.

No Call/No Show will result in the total cost of service scheduled.

Late Arrivals: Your session may be shortened to accommodate clients scheduled following your appointment. Depending upon your arrival time, your therapist will determine if there is enough time remaining to start your treatment. Regardless of the length of treatment, you will be responsible for the price of the full service.

Shipping & Delivery

At this time, Wellness Origin ships merchandise to locations within the continental United States. We are currently not shipping internationally or outside the continental US. The risk of loss and title for all merchandise ordered on this website pass to you when the merchandise is delivered to the shipping carrier.

We endeavor to ship all orders within 24-48 hours of receipt. However, if we are out of stock on an item, it may take a few additional days to ship the merchandise. Orders placed Friday afternoon through Sunday will be shipped the following Monday.

We will not be able to notify you via email when your order has shipped because of the multiple facilities our products are shipped from. Expect your products to arrive in multiple shipments. Upon receipt of your order, please inspect all items to ensure none were damaged during shipping. If there are damaged items, please refer to our Return Policy for the appropriate information.

All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Sales Tax

Wellness Origin charges sales tax for merchandise ordered on this website based on the applicable state sales tax rate and the location to which the order is being shipped.

Return Policy

All unopened retail products purchased within 30 days can be returned for a credit to your account to be used at Wellness Origin. No returns or refunds given for food products. Please call customer service at 317-979-1234 if there are any issues with perishable food purchases.

No Refunds: Cash, check, or credit card purchases are non-refundable.

Void Where Prohibited

Although the information on this website is accessible worldwide, not all products or services discussed in this website are available to all persons or in all geographic locations or jurisdictions. Purchase Policy

All payments through Wellness Origin are processed in USD. We accept cash, check or major credit cards for in-store purchases. Online purchases may be conducted using major credit cards. Gift cards are also issued and redeemed in USD.

All purchases made through Wellness Origin or wellnessoriginindy.com are subject to our return policy as listed above.

All services purchased expire in 1 year from the date of purchase.

Additional Terms & Conditions

We want to make sure that you have the most relaxing, enjoyable spa experience at Wellness Origin. To create an ambient environment for you, we respectfully ask that you follow our terms and conditions.

Please respect our quiet, tranquil environment to ensure the enjoyment and peace of all spa guests.

We accept payments and gratuity in cash, check and credit card. A minimum of 15% gratuity is courteous, as our spa practitioners spend additional time and effort to create an environment of maximum relaxation and peace for you.

We respectfully ask that you understand our policies – this allows us to maximize the visit of every client coming through our doors.

Liability Waiver

By purchasing a Wellness Origin product or service, I acknowledge that the treatment is for the intended purpose of assisting my personal health and beauty needs. Additional uses and outcomes of Wellness Origin products and services will be provided by the practitioner, and I will be asked to acknowledge such information as well.

I have read over the information on my specific products and services purchased through Wellness Origin, and I understand the benefits and surrounding information. Wellness Origin is not responsible for any injury, lost or stolen items or implications related to my experience at the spa. I assume liability for my actions and my experience at Wellness Origin Day Spa and Wellness Center outside of the stated perimeters.

Memberships

The following terms and conditions apply:

• Auto Payments: You will be charged each month on the first of the month. This charge will occur monthly, regardless of whether you show up for your monthly service or use the product discount.
• Appointment Scheduling: Appointments can be scheduled at our front desk, by phone, or online, per standard scheduling procedure of our Spa.
• Cancellation of Program: For cancellation, we require a 30 day notice, in order for our Spa staff to undergo standard cancelling procedure.
• Cancelling Appointments: Appointments which are cancelled can be rescheduled for another time slot. You will notify Wellness Origin at least 24 hours in advance in the event of an appointment cancellation. We will not honor a refund in the case of a late cancellation.
• Program Duration: There is no end date to the program. You are in the program until you choose to remove yourself.
• Non-transferable notice: Membership perks cannot be combined with any other offer. You receive one service at the membership discounted price each month, and this service may not roll over to upcoming months. Membership perks cannot be shared with other new or existing clients.

Refunds: All service transactions are non-refundable. Gift Certificates are non-refundable.

Booking an appointment at Wellness Origin, you are agreeing to the terms of our Cancellation and Refund Policy.

Text Messaging and Opt-In Data

We may send you text messages (SMS) for appointment reminders, order updates, promotions, or other service-related communications. You will only receive SMS messages if you have explicitly opted in (e.g., by providing your phone number and checking a consent box during booking, checkout, or sign-up).

Opting in is voluntary and not required to use our website or services. Message and data rates may apply. Frequency varies.

Opt-out: You may opt out of receiving text messages at any time by replying STOP to any message we send, or by contacting us at [email protected]. After opting out, you will receive one final confirmation message. Opting out does not affect other communications (e.g., email or account-related notices).
